The Financial Services Regulatory Authority of Ontario (FSRA) maintains a public registry of licensed, and formerly licensed health care service providers. The registry provides information on service provider facilities, licence status and conditions.
Statutory accident benefits claimants and auto insurers should check the registry, to quickly identify licensed service providers that can be paid directly by insurers for listed expenses.
Claimants and auto insurers can search for a licensed service provider by city, postal code, facility address and Principal Representative.
For a fee, you can obtain a certificate attesting to the status of a service provider licence.
To request a certificate of status, follow the instructions on the Public Registry of Licensed Service Providers.
