Transition period:
To allow time for transition, the previous version of Form 4 and the updated Form 4 can both be used until November 30, 2026. Effective December 1, 2026, the updated Form 4 must be used. Any Form 4s signed and dated on or after December 1, 2026, should be completed on the updated form in order to be accepted by the Plan Administrator.
