Transition period:
To allow time for transition, the current Form 3 and the updated Form 3 will both be available for use until November 30, 2026. Note: Effective December 1, 2026, the updated Form 3 must be used. Any Form 3s signed and dated on or after December 1, 2026, should be completed on the updated form in order to be accepted by the Plan Administrator.
