Auto insurers can use this form to submit an application (propose an innovation opportunity) to the Auto Insurance Activity TLE following FSRA Guidance No.GR0010APP and its Appendix 1 and 2. The innovation opportunity proposed for testing should fall within the scope prescribed by O. Reg. 704/21: Exemption Orders Under Section 15.1 of the Insurance Act.
