The 2019 HSP Annual Information Return (AIR) – Deadline Extended

We are monitoring the rapidly evolving circumstances around COVID-19. We understand the normal course of business has been disrupted and business continuity plans have been activated at many workplaces, including FSRA.

To support you in this difficult time, we are extending the deadline for filing your 2019 Annual Information Return (AIR) and regulatory fee to August 31, 2020. We thank those who have already filed.

FSRA continues operations and is consistently monitoring the health service provider sector while reviewing our work, stakeholder engagement activities and other commitments to prioritize activities. We are committed to communicating with you regularly. As we adapt to new norms, we ask for your patience and understanding where you experience delays.

The AIR is an annual questionnaire designed to collect specific information from service providers licensed by FSRA. It must be filed electronically, and may only be completed by the Principal Representative.

FSRA has reduced the time it takes for you to complete the AIR. As part of our burden reduction efforts, we have eliminated 30% of the questions compared to past AIRs, making it easier and saving you time.

To avoid a financial penalty, all licensed service providers are required to submit their AIR and regulatory fee by August 31, 2020. This includes those who did not conduct any business in 2019 but still held a service provider license at December 31, 2019.

Service providers in Ontario are required by law to submit an Annual Information Return. The process is as follows:

Step 1: Update your contact information

Phone number

Send your current phone number, along with all information below to: [email protected]. (Must be submitted by a principal representative.) Please include:

  • A short description of your request
  • Business name 
  • Licence number

Email address

  • Log in to your FSRA Account 
  • Select the “Change Email Address” 
  • Follow prompts

Your email will be updated immediately and confirmation sent to your new email address.

Ontario mailing address

  • Log in to your FSRA Account
  • Select the “Request a Change” 
  • Follow prompts

When complete, you will receive an email confirming your change. This notification will be also available in the Message Centre of your FSRA account."

Step 2: File the AIR 

Please note that when filing the AIR, you can save your application and return to it later, if more time is needed. FSRA recommends using Internet Explorer 11/Chrome/Edge to complete the AIR.

  • Make sure your information is up-to-date in FSRA Account. Note: All account updates must be complete in advance of completing your 2019 AIR. You will not be permitted to proceed with the AIR until all updates are submitted and accepted by the FSRA Account system. 
  • Select “I want to…Submit the AIR/Pay the Annual Regulatory Fee”
  • Follow prompts 
  • Check /update contact information
  • Review your application and fees. Note: If your regulatory fee is $5,000 or more, you must pay by certified cheque by June 15, 2020. All cheques and money orders are to be made payable to “Financial Services Regulatory Authority of Ontario”, or “FSRA”.
  • Select “Submit”
  • On the next page, select fee payment method
  • On the final page, select “View application” to print or download a copy of your application

Once your application has been received, you will receive an email confirmation.
